I really think this is great that they are seeking donations for funding their EMS program and all should support this service for the the community. However It really important that people have a strong understanding of the EMS services provided in San Felipe as it has a direct impact of level of care provided and overall patient outcomes. It was stated by person posting that the providers were Paramedics and that they were going to begin having Doctor accompany them on transports. Also I have been told that if the doctor is going to accompany the patient is because they are critical and the patients condition is poor. Let me see if I cn provide some insight and further understanding around this for everyone. Some may think they have some idea from watching the current show on TV which depicts some misonception of what emergency medicine is. My position also is from a place here in our community of San Felipe. After I decided to retire and move here I felt what was important was to also give back and pass on my knowledge and expertise in emergency medicine as well as the Fire service. This is when I developed San Felipe EMS program as well as supported the Fire Fighters Crossing Boarders group. San Felipe EMS was born out the desire to provide emergency medicine education and training to the local providers as well as support the current emergency response system and build upon it. With that it is important to know and understand what the cureent system looks like and how it functions. In San Felipe Cruz Roja Red Cross is primary EMS response provider. This is a non profit organization which depends on donation for support. I have been told that is the condition is critical or unstable then a Doctor will accompany patient during ground transport. This would be a patient that care is above the scope of practice of TUM providers. Also as an option for subscribers is a program that we have brought to the San Felipe community is a program called Medevac Baja which is a medical air evacuation program. In the event that a patients condition becomes serious or critical an air ambulance is flown in with a medical team: Doctor,Nurse or Paramedic and the patient is first stabilized at clinic and then medically transferred to medical team at airport and then flown out to location and hospital of choice. This procedure occurs when recieving medical Doctor determines the condition of patient is again serious or critical and calls our medical director and coordinates the emergency medical transfer to a US or canadian hospital and the pt is to be flown out. If the patient or family has not subscribed to this program of air medical transport then the patient would be transported by ground via ambulance again to Mexical or to US Boarder. I'm sure some people are saying well isn't it the same as back home. Some dont care and some really are OK with the current service that is provided. That is until the condition is serious or critical for themselves or family members. This is what the EMS system and training looks like over the boarder if interested. The level of training and licensure is based on emergency medical education and training. The County contract with EMS providers from Fire as well as private ambualnce providers who bid on provider contracts. In a urban community the system is operated under 911 and upon calling and implementing the emergency response a simultaneous dispatch occurs along with EMD pre arrival instructions as Fire based Paramedics as well as private ambulance are dispatched and responding. Majority of the time Fire based Paramedics arrive on scene first and implement care, and assessment and treatment until ambulance arrives on scene and based on patient medical condition the Fire Paramedic will either maintain care and accompany patient in ambualnce and continue care or transfer care to private paramedic and care and transport is continued to recieving emergency hospital. I understand this is a lot different than what occurs here in Mexico or in San Felipe Regardless of the difference in systems or level of care from San Felipe or Mexico to the US it is a understanding and an acceptance that yes it isnt the same nor do I personally want it to be. It is the intention of San Felipe EMS to work in conjunction with local EMS providers and medical care facilities and community and assit with education,training and emergency response and community programs to enhance the health and longevity or the citizens of San Felipe.
